How Do I Schedule An Appointment To See A Dietitian?

The fastest way to get an appointment is to have a referral faxed over to us by your primary care doctor or specialist. This usually requires that you have an appointment with your doctor to discuss your concerns. That information will help them send us the correct codes that we must use for billing your insurance.

Once we have the referral, you will be contacted for some initial information so we can check your insurance and make sure that you have coverage to see a Dietitian.

After coverage has been verified, you will be emailed paperwork to fill out. This is to give us a more accurate picture of your history as well as current questions and concerns.

This form must be returned to our office, along with other required paperwork, before your appointment can be scheduled.

Once we have all of the required paperwork, you will be called to schedule an appointment with either the first available dietitian or the one who has more experience in your area of concern.
